New Patient Registration

The Practice welcomes new patients. Please download the New Patient Registration form by clicking the link: GMS1 New Patient registration Form.

You will be asked to show two forms of ID when you register if possible (one photographic ID and one proof of address). We ask for this information to confirm that your details are correct when we register you, this ensures your medical record is sent through to us from your previous GP. Any copies of documents are disposed of confidentially when you are registered with us. If you are unable to show ID, you are still able to register with Penny Lane Surgery.

Newly registered patients will be asked to complete a questionnaire and will be offered a new patient health check with the Practice Nurse. Medical treatment is available from the date of registration.

We register patients who are living in L15 and some areas of L16, L17, L18 and L19. You can use this handy tool to check if your current address is within the practice boundary for Penny Lane Surgery.

 

Babies

New babies must be registered with the practice as soon after leaving hospital as possible and should complete the registration form above. Babies are usually seen at 6 weeks for a "6 week check" with a GP and again at 8 weeks by a Practice Nurse for the first of the routine childhood vaccinations.
